Target audience Cardiologists and others who care for patients with coronary artery disease. Educational objectives At the completion of this program, participants should be able to: Describe the pathobiology of acute coronary syndromes, and, specifically, the dynamics of spontaneous and mechanical plaque rupture in terms of activation of hemostasis and thrombosis. Discuss both the utility and limitations of currently available thienopyridines in percutaneous coronary intervention (PCI), and, in particular, the problem of clopidogrel resistance. Summarize the results of trials of abciximab in the setting of PCI for unstable angina/non-ST-elevation myocardial infarction and ST-elevation myocardial infarction (STEMI). Discuss new American College of Cardiology/American Heart Association guidelines concerning use of GP IIb/IIIa receptor antagonists in STEMI.
